1. I am the owner of the materials described below, including copyright, that Honey Pot Performance and Modern Dance Music Research and Archiving Foundation has requested permission to use and reproduce.
   
   
2. I hereby grant to the Honey Pot Performance and The Modern Dance Music Research and Archiving Foundation, and those authorized by the Honey Pot Performance and The Modern Dance Music Research and Archiving Foundation a royalty-free, irrevocable, and non-exclusive license to use, reproduce, distribute, and display the materials described below for all standard and non-marketing purposes, including but not limited to insertion on the Chicago Black Social Culture Map, archives, catalogues, education, exhibits, loans, promotions, public programs, public relations, publications, research, social media, and websites, without further compensation to me.
   
   
3. The materials may be used in whole or part for Honey Pot Performance and The Modern Dance Music Research and Archiving Foundation fundraising purposes (for example, in grant related or donor communications), but any commercial use shall require my prior written approval.
   
   
4. I shall indemnify and hold harmless the Honey Pot Performance and House Music Archive and its regents, directors, officers, employees, and agents, from any claim, demand, or injury arising out of the use of the materials as set forth herein, including but not limited to copyright infringement, defamation, invasion of privacy, or right of publicity.
   
   
5. Honey Pot Performance and The Modern Dance Music Research and Archiving Foundation’s right to use the Licensed Material as set forth herein shall be perpetual from the effective date of submission.
